Regarding Accommodations for Minor Guest's Stay

For guests under the age of 18 staying alone, we require that you submit a “Parental/ Guardian Consent Form for Minor Guest’s Stay” in advance.

※This form is required when a guest is a junior high school student or older but under the age of 18 and is staying alone
　or with other minors.
※A separate consent form must be completed for each minor guest and submitted to the hotel front desk upon check-in.
※If the hotel cannot confirm parental/guardian consent upon check-in, we kindly request that the completed form be
　submitted by the guardian via fax or mail after confirmation through a phone call.
※The personal information provided will not be disclosed or shared with third parties without prior consent,
　except as required by law.
